The investigation report on the major collapse accident at Three Gorges Minmetals Fujian Guangguang Co., Ltd. in Sanming, Fujian, which resulted in the deaths of three people, has been released.

On September 4, the Fujian Provincial Emergency Department released the investigation report on the "129" major collapse accident of Fujian Sanming Sangang Mingguang Co., Ltd.: On December 9, 2024, around 19:14, during the operation of removing hanging materials in the No. 4 silo of the sintering operation area of the iron department of Fujian Sanming Sangang Mingguang Co., Ltd. in Sanyuan District, Sanming City, Fujian Province, a sudden collapse of hanging materials occurred, resulting in the death of 3 people and a direct economic loss of 6.8276 million yuan. After investigation, it was determined that the "129" major collapse accident of Fujian Sanming Sangang Mingguang Co., Ltd. was caused by unauthorized command and operation by the workers, failure to implement safety measures during the clearance of hanging materials in the silo; inadequate "three simultaneous" management of safety facilities for the construction project, failure of the design unit to adopt the safety measures proposed by the safety facility design; inadequate performance of safety management personnel, failure to include the clearance of the silo in high-altitude operation management, failure to learn from similar accidents; inadequate supervision by government regulatory departments, and other factors resulting in a production safety responsibility accident. The direct causes of the accident were: unauthorized command, unauthorized operation, failure to enter the silo to clear hanging materials from top to bottom according to standard operating procedures and established construction plans, and unauthorized entry from the lower manhole of the silo for operation, leading to the accident. The accident investigation team made recommendations for dealing with 26 individuals and units responsible.
