The major explosion accident on May 27 at Shandong Youdao Chemical Co., Ltd. in Gaomi, Weifang, Shandong, was caused by defects in the technological process, unauthorized use of unqualified raw materials, improper selection of material conveying equipment, lack of risk control measures, formalities in hidden danger investigation and control, and illegal organization of project construction and production. The local party committee, government, and related departments failed to fulfill their duties properly, resulting in a significant production safety responsibility accident.
