On the 19th, NASA released an investigation report on Boeing's "Starliner" manned test flight mission, determining that the incident of astronauts being stranded at the International Space Station due to spacecraft malfunction was classified as the most severe "Class A" accident. NASA stated that the "Starliner" spacecraft lost maneuverability when approaching the International Space Station, causing related economic losses, and therefore classified this test flight as a Class A accident. According to NASA's accident classification, Class A accidents include incidents that result in death or permanent total disability of personnel, mission failures that result in direct property losses exceeding or equal to $2 million, destruction of manned spacecraft, and loss of control of spacecraft under unforeseen circumstances. The report noted that hardware malfunctions, qualification certification issues, and leadership errors interacted to create a risk situation during this test flight that did not meet NASA's manned spaceflight safety standards.
