UnitedHealth Group Incorporated (UNH.US) Q3 earnings beat expectations: decline in medical loss ratio boosts profits, full-year earnings guidance raised again.
UnitedHealth Group has announced its third-quarter results.
UnitedHealth Group Incorporated (UNH.US) reported third-quarter 2026 results and raised its full-year earnings guidance. Third-quarter adjusted earnings per share of $4.69 beat expectations, marking the third upward revision of full-year guidance this year to at least $20.50. Quarterly revenue was $112.694 billion, down 0.4% year-over-year, but still above the market expectation of about $111 billion. Operating profit was $6.563 billion, up 52% year-over-year, with a net profit margin of 3.6%. The company is the first major U.S. healthcare group to report third-quarter results, and its performance is seen as a bellwether for the industry.
On the earnings side, GAAP diluted earnings per share were $4.56, up 76% year-over-year; adjusted earnings per share, excluding items such as intangible asset amortization, asset disposals, and restructuring, were $4.69, up 61% year-over-year. According to analyst data compiled by LSEG, the market had previously expected adjusted earnings per share of $4.14, so the actual figure beat by $0.55. Net income attributable to common shareholders was $4.110 billion, up 75% year-over-year.
Cost control was the main driver this quarter. The quarterly medical loss ratio was 88.4%, better than analysts' expectation of 89.9% and the year-ago level of 89.90%, which the company attributed to insurance product design adjustments and new pricing. The company disclosed that the ratio included $610 million in net favorable prior-period reserve releasesfunds previously set aside for medical claims that were ultimately not used. Chief Financial Officer Wayne DeVeydt said the growth rate of medical costs "remains elevated, but is very stable." The quarterly operating expense ratio fell to 12.8% from 13.5%; operating cash flow was $5.1 billion, 1.2 times net income.
By segment, the insurance unit UnitedHealthcare reported revenue of $86.330 billion, down 0.9% year-over-year, while operating profit rose to $2.831 billion from $1.805 billion a year earlier, with margin increasing to 3.3% from 2.1%, serving 48.3 million members. The health services unit Optum reported revenue of $66.883 billion, down 3.3% year-over-year, while operating profit rose to $3.732 billion from $2.510 billion; within that, Optum Health saw revenue fall 10% year-over-year to $22.910 billion due to reducing services to patients in markets lacking a sufficient value-based care foundation, but operating profit rebounded to $735 million from $41 million.
According to reports, Optum Health is contracting: the company said earlier this year that it would cut about 20% of its clinics, or about 550 locations, most of which was completed in the first half, with the process to end by the end of 2026. DeVeydt said the goal is to transfer these clinics to new owners rather than close them, and that "at this stage we have no plans to close any." He also said Optum Health's full-year operating profit will exceed $2.2 billion.
On membership, the company continued to proactively shrink its scale. Employer and individual business decreased by 165,000 people this quarter, while community and state business fell by 85,000 people due to Medicaid eligibility changes; the company disclosed that since the end of 2025, the number of senior members served through Medicare Advantage and related programs (including complex populations covered by Medicaid) has decreased by 935,000. The company said that in the 2027 Medicare Advantage star ratings, at least 69% of its members are in plans rated four stars or above, "in line with expectations," and noted that 11 of 12 HEDIS quality metrics improved. However, the federal government last week downgraded the quality ratings of some UnitedHealthcare plans, and analysts at Evercore ISI and TD Cowen estimated this could reduce the company's 2028 revenue by more than $1 billion. DeVeydt said membership is expected to return to growth in 2027: "Is growth under consideration? We believe so."
On guidance, the company raised its full-year 2026 adjusted earnings per share guidance to at least $20.50 (previously $19.50-$20.00), and raised its GAAP diluted earnings per share guidance to at least $19.50. This is the company's third upward revision of full-year guidance this year. According to LSEG data, analysts had previously expected the company's full-year 2026 earnings per share to be $19.86, and the lower end of the new guidance is already above consensus.
UnitedHealth Group Incorporated Chief Executive Officer Stephen Hemsley said the third-quarter results "reflect the discipline and execution the team demonstrated across every part of the business." The company also listed a number of initiatives focused on affordability, prior authorization reform, and drug pricing transparency, including investing $1 billion in the United Health Foundation, eliminating 30% of existing prior authorization volume by the end of 2026, and passing 100% of drugmaker rebates to customers starting in January 2028.
This round of guidance increases is the latest step in the company's transformation process. Over the past two years, UnitedHealth Group Incorporated suffered the largest cyberattack in the history of the U.S. healthcare industry, failed to meet financial targets and was at one point forced to withdraw its earnings guidance, and the company also replaced several senior executives and exited some health insurance product lines. The company had previously said that its Optum unit faces regulatory and cost challenges, with related impacts hitting the subsidiary by about $11 billion over three years. According to Bloomberg, the company is also seeking private equity assistance to operate some doctor clinics; as federal policy changes such as 2027 Medicaid work requirements take effect, uncertainty will rise further.
Driven by the results and the guidance increase, UnitedHealth Group Incorporated shares rose in premarket trading on October 13. As of press time, the stock was up 2.5% premarket to about $388. The earnings report also boosted the share prices of UnitedHealth Group Incorporated's peers, including Humana (HUM.US), CVS Health (CVS.US), Centene (CNC.US), Alignment Healthcare (ALHC.US), and Elevance Health (ELV.US).
